DOT physicals are usually not covered by insurance. The cash rate for an in-clinic DOT is $125.00.

How long before my current certificate expires?
This information should be written on your card. However, 2 years is the longest certification given.
What if I wear glasses or hearing aids?
Bring them along! This will be taken into account during your exam.
What if I have high blood pressure, or another chronic condition?
This will be taken into account during the exam. Chronic conditions can lead to reduced certification periods.
You will be asked to fill out Medical History forms upon arrival. If applicable, be sure to have these forms filled out prior to your exam:

▸ Skilled Performance Evaluation certificate - if known limb differences
